Comfort - There are generally two types of bluetooth headsets, those that loop around the ear and others that plug into the ear. The loop type earpieces, while usually more comfortable, tend to slip and are not quite as secure as the type that offer earplugs. But if you spend a great amount of time on your phone while on the road, then you might want to spend that little extra for the ability to almost instantly connect and disconnect to your phone.

If you're only going to use it to get from your home to your office you won't need as much battery life as, for example, a foreman on a construction site who needs to be in constant contact with several different job sites.

Clarity - Think about where you might be using the bluetooth headset. Will you mostly be inside a quiet office? Or will you instead be in a car for the majority of the time, being able to mostly drown out much of the traffic noise, but not completely? Or do you happen to be the construction site foreman who will most likely require filters or special features in order to allow you to better hear others? If you are concerned about reducing background noise, search for a headset that has mechanisms of noise cancellation or echo reduction technology.

Distance - Bluetooth headsets can be used to connect to portable mp3 devices as well as your mobile phone. Because they operate on a very low frequency, keep in mind the distance between your bluetooth headset and the device you'll be connecting to. If you typically leave your phone on your desk while you move around the building, you may need to go for a higher end unit with more power at the antenna.
